Reports for any diagnostic imaging including CT scans, PET scans, MRI's, X-rays, Ultrasounds, etc. We do not need the actual film, just the reports. |
| Copies of all blood work. | |
| Copies of summary reports from any specialists you have visited for evaluations. | |
Medicare
Dr. Ross is opted out of Medicare. If you are on Medicare you will need to sign a contract that Medicare requires you to sign. We will provide you with that contract. Please note that it is illegal for you to bill Medicare for charges from this clinic as we are opted out and do not participate in the Medicare system.
Insurance
Practitioners in the clinic do not take medical insurance. Payment is due at the time of service.
First Appointment Deposit
There is a $200.00 deposit to schedule an initial appointment of one hour or longer. If a patient needs to change or cancel their appointment, they need to give the office 2 business days notice. This means if the appointment is on Friday, the patient would need to call by Tuesday night to have their deposit refunded. This would give us two full days to fill that appointment.
Fragrance Free Environment
Please note there is a NO SCENT/FRAGRANCE policy at the clinic. Please refrain from wearing any colognes or perfumes. When you visit the clinic please refrain from using any scented hair products, laundry products, soaps, oils or lotions.
Parking
Parking is available under the building in the spaces labeled 'Highline Medical Parking Only (24 Hours)'. Take the elevator to the first floor.
